Your Extended Health Insurance Plan

Note: This document is prepared for your information, as a general guide to the most common practices of Extended Health Plans. Individual coverages vary widely, and it is essential that you obtain full details through your employer. We will be happy to provide you with documentation to submit to your insurance company before treatment commences.

This bulletin explains which services are generally covered under your employer’s extended health care plan. Individual plans vary and cover different services. Some services may be covered to a dollar value maximum per year. You should check with your employers Human Resources department or read the documentation the insurance company provides for exact details.

The Ontario Health Insurance Plan (OHIP) does not cover our services. If you do not have an extended Health Insurance plan (EHIP), you will have to pay for services yourself. For those without an EHIP, many health care services we offer that would normally be covered by an EHIP are tax deductible. Your tax accountant should be consulted for details.

Everything we do at my Chrysalis Medi-spa Salon enhances your health and well-being. However, not all services are covered under EHIP. Generally, Salon and cosmetic services are not covered, medical services are. Because we have only highly trained Registered Massage Therapists (RMT), massage services are generally covered.

If you have an EHIP, we will provide you with receipts for you to submit to the insurance company. We ask that you pay for services when they are delivered and claim the reimbursement yourself through your employer. If you do not have an EHIP, we will, upon request, provide receipts for you to submit with your tax return.
